Hridayapoorvam follows the story of Sandeep Balakrishnan (played by Mohanlal), a rich and irritable businessman who manages cloud cuisine in Kochi, Kerala. Despite his wealth, he lives in isolation and neglects personal relationships.
After having undergone a heart transplant, he returns to work and rejects the emotional weight that often accompanies a such procedure that changes life. His indifference even irritates those around him, who expect Sandeep to be grateful and introspective.
During the following weeks, Sandeep is invited to attend the engagement ceremony of the daughter of her heart donor, Haritha, in Pune. Accompanied by his assistant, Jerry (Sangeeth Pratap), Sandeep disturbs the donor’s family with his cold attitude and his apparent contempt for the sentimental meaning of cardiac transplantation.
However, the ceremony quickly takes a dramatic turn when the groom suddenly cancels the commitment, humiliating both Haritha (Malavika Mohanan) and his mother, Devika. In the middle of chaos, Sandeep enters an altercation and undergoes a back injury.
Unable to travel because of his condition, Sandeep is forced to extend his stay with Haritha and her family. Initially away and detached, he gradually begins to soften in response to the heat and kindness shown by Haritha and Devika.
Thanks to a continuous interaction with them, Jerry and others, Sandeep begins to evolve from a man who feared the emotional link with a person who opens slowly and kisses him.
Devika (Sangita Madhavan Nair) accepts Sandeep as part of the family, which prompted him to confide in emotions that he had long suppressed.
Meanwhile, Haritha, still in shock from her broken commitment, finds comfort and strength in the company of Sandeep. Encouraged by him, she begins to move forward and understand that greater things can expect.
In the last moments of Hridayapoorvam, Sandeep is no longer a guest but has become a permanent part of the life of Haritha and Devika. As they spend peaceful moments together in their courtyard, Sandeep realizes that a heart is not only an organ but also a symbol of love, connection and belonging.
